Tax Preparation Document Checklist

To help ensure your tax return is prepared accurately and efficiently, please gather the documents that apply to your situation. Not all documents listed below may apply to every taxpayer.

Personal Information

Please provide the following basic information:

  • Government-issued photo ID

  • Social Security Number (SSN) or ITIN for you, your spouse, and dependents

  • Date of birth for all individuals on the return

  • Last year’s tax return (if available)

  • Bank account and routing number for direct deposit of refunds

Income Documents

Bring all documents that show income received during the tax year.

Common income documents include:

  • Form W-2

  • Form 1099's

Self-Employment or Business Documents

If you are self-employed or own a business, please provide:

  • Business income records

  • 1099 forms received

  • Profit and loss statement

  • Business expense records

  • Receipts for business purchases

  • Mileage records

  • Home office expenses

  • Business bank statements

  • Business credit card statements

  • Previous year business tax return

Dependent & Family Information

If claiming dependents, please bring:

  • Social Security numbers for dependents

  • Birth certificates (if requested)

  • Childcare provider information

  • Childcare expense records

  • Form 1098-T for education expenses

  • Student loan interest statement (1098-E)

Health Insurance Documents

If applicable, please provide:

  • Form 1095-A – Health Insurance Marketplace statement

  • Form 1095-B or 1095-C – Health coverage information

Homeownership & Property Documents

If you own a home or property, bring:

  • Form 1098 – Mortgage interest statement

  • Property tax statements

  • Mortgage insurance statements

  • Records of home purchase or sale

  • Home improvement records (if applicable)

Credits & Deductions

Documents that may qualify you for tax credits or deductions include:

  • Childcare expenses

  • Education expenses

  • Charitable donation receipts

  • Medical expense records

  • Property tax payments

  • Retirement contributions

  • Energy-efficient home improvement receipts

Other Important Documents

If applicable, also bring:

  • Identity Protection PIN (IP PIN) from the IRS

  • IRS letters or notices received

  • Estimated tax payment records
